Any individual wishing to attend the 13th Annual Advancing Practice, Instruction, and Innovation through Informatics (APIII 2008) Conference must register online, by mail or by faxing the registration form (PDF) . Registration forms submitted without full payment cannot be processed and will be returned.

We strongly encourage early registration, as space is limited for this meeting.  Registration forms submitted without full payment cannot be processed. Onsite registration will be available at the Marriott City Center, Pittsburgh, October 18 – 22, 2008.  If you do not receive a confirmation, contact Wendy Frank by email at frankwm@upmc.edu.
 
Important Deadlines

June 30, 2008: Abstract Submission
July 15, 2008: Early Registration Deadline
September 1, 2008: Deadline for Cancellation of Advance Registration to receive registration refund, less processing fee of $100
September 22, 2008: Hotel Reservation
 
Payments
Payment must accompany each registration in order to hold your place. Online registrations require the use of an accepted credit card (Visa, MasterCard, and American Express only) Registration forms submitted without full payment cannot be processed. 

Cancellation / Refunds.  
Request for refunds must be made in writing and sent by mail, e-mail or fax. A processing fee of $100 will be applied to refunds requested by September 1, 2008.  No refunds can be made after September 1, 2008. Cancellation requests must be made to Wendy Frank by email: frankwm@upmc.edu or by fax (412) 623-2814.
